The study of blood serum trace elements in comparison with the structural and functional characteristics of goiter and the carriage of antithyroid antibodies in some regions of Russia

Abstract

The study of the sufficiency with essential trace elements in the context of thyroid pathology is especially relevant, since the development and progression of thyroid diseases have various pathogenetic mechanisms, which largest proportion is provoked by insufficient intake of key trace elements, primarily iodine.

The aim of the research was obtaining primary data on the sufficiency with essential trace elements of the population in the some regions of the Russian Federation, comparing the data obtained with the prevalence of goiter and the carriage of antibodies to thyroid peroxidase (AT-TPO).

Material and methods. The Cross-Sectional Population Study was conducted in medical institutions of the Republics of Crimea, Republics of Tuva and Bryansk region (persons applied for outpatient consultations). The scope of the study was 387 people aged 18 to 65 years (the average age was 40±5 years). All of them underwent: collection of medical history, endocrinologist examination with palpation of the thyroid gland (TG), ultrasound examination of TG, blood sampling to assess the levels of thyroid-stimulating hormone (TSH), AT-TPO, iodine, selenium, zinc. Ultrasound of the TG was performed in the supine position using a portable ultrasound machine LOGIQe (China) with a multi-frequency linear probe 10–15 MHz. During the study, the volume of the thyroid gland, the presence of nodular formations and their characteristics according to the TIRADS classification, TG structure and its echogenicity were assessed. Iodine, selenium and zinc level were measured using tandem mass spectrometry with ionization in inductively coupled plasma (Agilent 8900 ICP-MS Triple Quad). TSH and Ab-TPO were determined by chemiluminescent immunoassay on the Architect i2000 automatic analyzer.

Results. All regions are comparable in terms of iodine availability, but differ in the severity of natural iodine deficiency. The median concentration of iodine in blood serum practically coincided in all three regions, amounting to 39.8 µg/l in the Republic of Crimea; 38.8 µg/l in the Republic of Tyva and 43.4 µg/l in the Bryansk region. Comparing these results with foreign data, as well as with our results on urinary iodine level, obtained by routine cerium-arsenite method, we can assume that iodine content in blood serum corresponded to the lower limit of the normal target values.

The median concentration of selenium in blood serum also practically coincided in all three regions, amounting to 68.2, 72.1 and 62.8 µg/l, respectively, and probably meets the criteria for optimality. The median serum concentration of zinc in Bryansk region residents corresponded to a deficiency (491.3 µg/l) and was significantly lower compared to the indicator in the Republics of Crimea and Tyva (1633.2 and 1667.6 µg/l, respectively, values above optimal). Among the examined, the proportion of people with AB-TPO carriership was 23.5% (Republic of Crimea – 20.9%, Republic of Tuva – 26.4%, Bryansk region – 20.7%), with impaired thyroid function – 9.6% (6.2% – hypothyroidism, 3.4% – thyrotoxicosis). The frequency of goiter varied from 10 to 13.8%. There were no significant differences in the frequency of occurrence, ultrasonic characteristics of goiter and thyroid nodules by regions.

Conclusion. However, the results obtained do not allow to unequivocally judge the absence of micronutrient deficiency in the population of the regions of the Russian Federation, since used the ICP-MS method requires the development of our own reference values of trace elements; in the absence of the latter, data from similar foreign studies were taken as standards. In addition, in further studies it will be important to take into account additional criteria for assessing trace element deficiency, for example, activity of superoxide dismutase and alkaline phosphatase as indirect markers of zinc deficiency, etc. Correlation between blood serum concentration of trace elements and the frequency and structural characteristics of goiter, the carriage of Ab-TPO, and dysfunction of the TG was not revealed.

Всем включенным в исследование лицам выполнено ультразвуковое исследование (УЗИ) ЩЖ с использованием портативного ультразвукового аппарата LOGIQe (Китай) с мультичастотным линейным датчиком 10-15 МГц в положении лежа. Оценивали объем и структуру ЩЖ.

Объем ЩЖ рассчитывали по формуле:

Vщж = [(Шпр × Дпр × Тпр) + (Шл × Дл × Тл)] × 0,479,

где Vщж - объем ЩЖ; Шпр - ширина правой доли ЩЖ; Шл - ширина левой доли ЩЖ; Дпр - длина правой доли ЩЖ; Дл - длина левой доли ЩЖ; Тпр - толщина правой доли ЩЖ; Тл - толщина левой доли ЩЖ.

Элементный анализ сыворотки крови методом масс-спектрометрии с индуктивно связанной плазмой

Определяли микроэлементы с использованием тандемного трехквадрупольного масс-спектрометра с ионизацией в индуктивно связанной плазме. Для максимального исключения интерференций был использован комбинированный подход: Zn и I определяли в реакционно-столкновительном режиме с гелием и детектировали ионы с m/z 66 и 127 соответственно; Se определяли в реакционном режиме с кислородом с последующим детектированием иона SeO+ на m/z 96 для отделения от плазменной интерференции. Такой подход позволил максимально исключить возможные интерференции ионов 40Ar26Mg+, 32S16O18O+ и 32S34S+ для Zn, а также 40Ar2+ для Se. Интерференции для 127I в физиологических образцах не характерны.

Перед непосредственным определением, образцы размораживали, аликвоту сыворотки (0,5 cм3) переносили в герметично закрывающиеся фторопластовые сосуды и проводили микроволновое разложение проб в режиме 20 мин - 120 °С, 10 мин - 150 °С, с добавлением 2,5 cм3 охлажденной (-10 °С) концентрированной азотной кислоты (69%, Merck) и 0,5 cм3 охлажденной (+4 °С) концентрированной перекиси водорода (30% Sigma-Aldrich). После разложения образец разбавляли деионизированной водой (Milli-Q) для снижения концентрации азотной кислоты до 10% (об.) и вводили при помощи автосемплера в масс-спектрометр. В качестве внутреннего стандарта использовали сульфат церия (IV), который в одинаковых количествах добавляли в каждый образец.

Статистический анализ. Данные представлены в виде абсолютных значений и процентов от общего количества. Для описательного статистического анализа концентрации микроэлементов в сыворотке крови были использованы значения медианы и частотного распределения. Значимость различий оценивали с использованием непараметрического критерия Манна-Уитни.

Результаты и обсуждение

Медианная концентрации I в сыворотке крови обследованных практически совпадает во всех 3 регионах (рис. 1). Сравнивая результаты с зарубежными данными (диапазон референсных значений в сыворотке крови для метода ИСП-МС составляет от 39 до 118 мкг/л [16, 17], а также с результатами определения йода в моче, полученными нами рутинным церий-арсенитным методом (мКЙМ составила: в Республике Крым - 97 мкг/л; в Республике Тыва - 153 мкг/л; в Брянской области - 98,3 мкг/л [7, 10, 11]), можно предположить, что показатели йода в сыворотке крови соответствуют нижней границе нормы.

Частота зоба у взрослых по данным УЗИ варьировала от 10% (Республика Тыва) до 13,8% (Брянская область).

Практически каждый 4-й обследованный был носителем АТ-ТПО (рис. 2). В целом нарушение функции ЩЖ выявлено у 9,6% обследованных: у 6,2% диагностирован гипотиреоз и у 3,4% - тиреотоксикоз.

Выявляемость узлов, соответствующих градации 1-3 и 4 и выше, составила: в Республике Крым - 12,3% (в том числе узлового зоба - 13,3%); Тыва - 10% (в том числе узлового зоба - 40,6%); в Брянской области - 13,8% (в том числе узлового зоба - 56%).

Таким образом, по частоте встречаемости, ультразвуковым характеристикам зоба и узловых образований ЩЖ существенных различий у обследованных в разных регионах не выявлено. В основном ультразвуковые характеристики узловых образований согласно стандартизированной системе описания протокола УЗИ ЩЖ (European Thyroid Imaging Reporting and Data System) соответствовали коллоидным зобам - EU-TIRADS 2 (рис. 3).

Показатели медианной концентрации Se в сыворотке крови соответствовали целевым значениям и практически совпали во всех 3 регионах (рис. 4). По данным зарубежных исследований референсный диапазон концентраций селена в сыворотке методом ИСП-МС варьирует от 30 до 123 мкг/л [16, 17].

По данным зарубежных исследований, референсный диапазон концентраций Zn в сыворотке крови для метода ИСП-МС составляет от 600 до 1200 мкг/л [16, 17]. Медиана концентрации Zn в сыворотке крови обследованных в Брянской области оказалась значительно ниже, что соответствует дефициту, по сравнению с таковой у жителей Республики Крым и Тыва (значения выше оптимальных) (рис. 5). Полученные результаты можно объяснить как особенностями питания населения регионов, так и содержанием микроэлементов в почве и воде по каждому региону, в том числе других тяжелых металлов, которые потенциально могут влиять на метаболизм Zn.

Корреляционной зависимости между концентрацией микроэлементов в сыворотке крови и частотой зоба, носительством АТ-ТПО, а также нарушением функции ЩЖ выявлено не было.

Изучение обеспеченности эссенциальными микроэлементами жителей ряда регионов РФ, имеющих достаточно высокую распространенность заболеваний ЩЖ по данным Росстата (https://rosstat.gov.ru/), особенно актуально, поскольку показана взаимосвязь между содержанием и метаболизмом I, Se, Zn, Fe и других микронутриентов и риском развития диффузного и узлового зоба, дефицит ряда эссенциальных микроэлементов, включая I, Se и Zn, может повысить риск развития зоба. В то же время имеющиеся данные во многом противоречивы. Для оценки достоверности рассматриваемой проблемы требуются исследования с применением новейших методов определения микроэлементов в биологических образцах. Наиболее точным в настоящее время методом определения общего содержания элементов в образцах крови и мочи является ИСП-МС.

Результаты проведенного нами исследования не позволяют однозначно судить об адекватной обеспеченности эссенциальными микроэлементами населения регионов РФ, поскольку используемый нами метод ИСП-МС требует разработки собственных референсных значений. Согласно данным многих исследований в отношении Zn и Se важно также учитывать дополнительные маркеры обеспеченности, такие как активность супероксиддисмутазы, щелочной фосфатазы, глутатионпероксидазы и др., что требует дальнейшего более детального и углубленного изучения рассматриваемой проблемы.
