Assessment of the role of nutritional and genetic determinants in the formation of the risk of diseases associated with folate cycle disorders in the population of the Omsk region
AbstractA number of essential nutrients are involved in the folate cycle, and its effectiveness depends on the sufficient intake of them. In addition, polymorphic variants of the methylenetetrahydrofolate reductase (MTHFR), methionine synthase reductase (MTRR) and methionine synthase (MTR) genes affect a wide range of biochemical reactions of the folate cycle and should also be considered as a risk factor for the development of a number of diseases.
The purpose of this research was to study the prevalence of these risk factors.
Material and methods. The prevalence of polymorphisms of the folate cycle genes: C677T polymorphism of the MTHFR gene and A66G polymorphism of the MTRR gene in a random stratified (by sex and age) sample of the adult population of the Omsk region [n=139, 51 men, 88 women, aged 18 to 75 years, median age 37 (26; 48) years] was studied. The identification of polymorphisms was carried out by the method of allele-specific polymerase chain reaction with an electrophoretic detection scheme. Using the food intake frequency questionnaire, the dietary intake of nutrients involved in the folate cycle was determined: B vitamins (B6, B2, B9, B12), methionine, choline, in a representative stratified sample of residents of the Omsk region [n=421, 177 men, 244 women, aged 18 to 83 years, median age 37 (23; 57) years].
Results. MTHFR genotypes (A222V С677T C>T) were distributed as follows: CC-type – 51.3%, CT – 41.0%, TT – 7.7%; MTRR genotypes (I22M A>G): AA type – 57.9%, AG – 30.3%, GG – 11.8%. The analysis of actual nutrition showed consumption below the recommended dietary intake of folates in 88.2% persons, vitamin B2 and choline – in 40.5%, vitamin B6 – in 29.2%, methionine – in 22.0%. Vitamin B12 intake was within the recommended range.
Conclusion. The totality of the data presented indicates the combined influence and wide distribution of factors that determine the low efficiency of the folate cycle, and, as a result, a high risk of developing a characteristic pathology for the adult population of the region, which determines the need and priorities for prevention measures, including healthy nutrition.

Современная наука располагает обширными данными о взаимосвязях недостатка компонентов фолатного цикла с развитием социально значимых заболеваний, в том числе определяющих высокий уровень показателей смертности. В этой связи в ряде работ проведено изучение полиморфизмов генов фолатного цикла, их ассоциаций с патологией, а также оценка распространенности полиморфных вариантов в разных этнических группах [12, 24].
Аллель 677T гена MTHFR имеет широкое распространение, но имеют место значительные межрасовые и межэтнические различия. Чаще всего этот аллель встречается в европейских (0,28-0,47) и азиатских (0,18-0,41) популяциях, в то время как у чернокожих африканцев и аборигенов Австралии и Шри-Ланки гораздо реже (до 0,15) [25]. Полученные в нашем исследовании результаты встречаемости полиморфных аллелей 677Т согласуются с данными аналогичных исследований, касающихся большинства европеоидных популяций (в том числе Россия 0,28) [12, 24], а также жителей Сибири (0,33) [25].
Частота встречаемости аллелей G гена MTRR также широко варьирует в разных популяциях (Россия - 0,64, Кавказский регион России - 0,35, Италия - 0,52, Франция - 0,57, Бразилия - 0,07) [24]. В настоящем исследовании она оказалась несколько ниже, чем в европеоидных популяциях, но вполне соотносилась с данными, приведенными в аналогичных работах [12, 24].
Одной из частых причин недостаточной обеспеченности исследуемыми нутриентами является их сниженное поступление с пищевыми продуктами как по причине невысокого содержания в продуктах, так и вследствие изменения структуры питания с отклонением от принципов здорового питания. При этом данные анализа фактического питания и пищевых привычек позволяют косвенно оценить обеспеченность ими на популяционном уровне. Существенное значение для получения достоверных результатов в исследованиях фактического питания имеет качество базы данных химического состава потребляемых пищевых продуктов. В результате проведенных ранее исследований состава пищевых продуктов на территории Омского региона было установлено, что в большинстве образцов моркови, печени говяжьей, капусты имелись невысокие уровни содержания фолатов относительно средних, приводимых в справочных таблицах химического состава, при этом в ряде продуктов растительного происхождения они вполне соответствовали справочным данным [15]. Известно также, что термическая кулинарная обработка пищевых продуктов может приводить к разрушению 70-90% витамина B9 [14]. Эти результаты были учтены в базе данных, использованной для расчетов в настоящем исследовании.
Оценка фактического питания жителей российских регионов показывает, что не все население получает с продуктами необходимое количество витамина В2. Такие данные имеются для республики Татарстан (медиана суточного потребления 1,1-1,2 мг) [26], Москвы и Московской области (1,72±0,06 и 1,62±0,07 мг) [27]. Исследования, проведенные среди подростков и взрослого населения Западной Сибири, также выявили потребление ряда витаминов с рационом ниже рекомендуемых уровней (В2 - у 34,0%, В6 - у 7,2%, В9 - у 89,2%) [14].
Оценка витаминного статуса жителей Омской области продемонстрировала высокую распространенность гиповитаминоза В9 (у 63,2%) и В6 (49,7%); недостаточная обеспеченность витамином В12 выявлена у 13,2% [13]. Данные по изучению витаминного статуса жителей Москвы и российских регионов свидетельствуют о высокой распространенности недостатка витамина B2 (у 45% обследованных), витамина B9 - у каждого 5-го обследуемого и оптимальной обеспеченности витамином B12 [27]. В целом эти данные коррелируют с полученными нами результатами оценки фактического питания.
Заключение
Изучение частоты генетических полиморфизмов, обусловливающих нарушения обмена фолатов, у взрослого населения Омской области показало, что большинство обследуемых составляли лица с гомозиготным состоянием по нейтральным аллелям генов MTHFR (51,3%) и MTRR (57,9%). В то же время выявлена высокая частота (как в группе мужчин, так и женщин) встречаемости гетерозиготных генотипов, несущих неблагоприятные варианты полиморфизмов генов MTHFR (41,0%) и MTRR (30,3%), а также их сочетание (17,1%), что может вносить существенный вклад в повышение уровня гомоцистеина, особенно на фоне низкого содержания фолата в плазме крови. Этот риск особенно высок у гомозиготных носителей полиморфных аллелей.
Проведенный анализ фактического питания показал, что существенный вклад в возможность развития нарушений фолатного цикла вносит также недостаточное потребление фолатов, витаминов B2 и B6 вследствие чего формируется низкая обеспеченность ими.
Таким образом, оценка роли пищевых и генетических детерминант в формировании риска заболеваний, связанных с нарушением фолатного цикла, у населения Омской области показала наличие их сочетанного влияния и широкого распространения, что определяет низкую эффективность фолатного цикла у значительной доли населения и, как следствие, высокий риск развития соответствующей патологии.
