Effect of dietary supplements and whey protein on muscle mass and strength of the operated limb after anterior cruciate ligament reconstruction: a systematic review

Abstract

Muscle weakness and atrophy of the quadriceps muscle after anterior cruciate ligament reconstruction may persist for up to 6 months after surgery and cause re-injury to the ipsilateral or contralateral limbs. Many authors state that adequate nutritional status during the rehabilitation period can contribute to faster postoperative recovery of muscle mass and strength of the lower limb, be an adjunct to exercise, or serve an alternative treatment strategy.

The purpose of the research was to conduct a systematic review of the literature and evaluate the degree of influence of dietary supplements on muscle mass and strength of the operated limb after reconstruction of the anterior cruciate ligament (ACL).

Material and methods. The search for articles was carried out in international databases PubMed, Google Scholar, Cochrane Library. The time frame of the search was 22 years. To be included in the review, studies had to meet the PICOS criteria: participants were men and women over 18 years of age after ACL reconstruction; intervention – supplementation after and/or before and after ACL reconstruction; comparison – placebo group or no supplementary interventions; results – assessment of the dynamics of changes in muscle mass (cross-sectional area, muscle thickness or size of skeletal muscle fibers) and / or maximum strength (dynamic or isometric) of the operated limb; research design – randomized controlled trials (RCT). The quality of selected RCTs was assessed using the Cochrane Collaboration Risk of Bias (RoB 2) tool.

Results. A total of 1397 articles were found after searching in the databases. This systematic review included 6 RCTs with a total number of patients 186. The following dietary supplements were used: leucine – 1 article, glucosamine – 1 article, creatine – 1 article, vitamins E and C – 2 articles. One study evaluated the effect of whey protein supplementation in combination with neuromuscular electrical stimulation on isometric strength of the operated limb. When comparing the data of 3 groups in the pre- and postoperative periods, there were no statistically significant differences between the groups. When considering the protocols for taking dietary supplements based on creatine, glucosamine, vitamins E and C, none of the studies demonstrated statistically significant improvements in the parameters of maximum strength or muscle hypertrophy of the operated limb in the main groups compared with the control groups. Also, no results have been obtained indicating that these dietary supplements can slow down muscle atrophy after surgery. In another study that evaluated the effect of leucine supplementation, by the end of the rehabilitation program, the muscle strength of the operated limb tended to increase more in the leucine group than in the placebo group, but without a statistically significant difference. At 10 cm from the patella, the femoral circumference of the operated limb in the leucine group increased more than in the placebo group, and the differences were statistically significant (р=0.009). Two studies were rated as high risk of bias, three as moderate risk, and one as low risk.

Conclusion. None of the supplements used had a statistically significant effect on the muscle strength of the operated limb after ACL reconstruction. In terms of thigh muscle hypertrophy, the only significant improvement was associated with leucine supplementation and an increase in thigh circumference at a distance of 10 cm from the patella.

Лейцин

Некоторые данные свидетельствуют о том, что дополнительный прием лейцина может частично и временно защищать скелетные мышцы во время отсутствия нагрузок, сохраняя анаболизм и смягчая снижение их массы [40, 41]. В статье E. Laboute и соавт. [25] к концу реабилитационной программы мышечная сила имела тенденцию к более выраженному увеличению в группе пациентов, принимавших лейцин, по сравнению с пациентами, принимавшими плацебо, но без статистически значимых различий. При измерении обхвата бедра на расстоянии 15 см от надколенника показатели оперированной конечности в основной группе статистически значимо не отличались от параметров в группе контроля (р=0,38). В то же время на расстоянии 10 см от надколенника обхват бедра поврежденной конечности у участников, принимавших лейцин, увеличился больше, чем в группе участников, принимавших плацебо, при этом различия показателей были статистически значимыми (р=0,009). Статистически значимых различий между группами при измерении результатов прыжка в длину на одной ноге не выявлено. Частично схожие данные были получены в двойном слепом плацебо РКИ A.F. Aguiar и соавт. [42], где 20 молодых здоровых участников (возраст 22±2 года) принимали по 3,0 г лейцина в день в течение 8 нед. Основным выводом этого исследования было то, что дополнительный прием лейцина не увеличивал мышечную массу и силу во время силовых тренировок у ранее не тренированных молодых людей. В свою очередь, I.T. De Andrade и соавт. [43] сообщили, что высокие дозы лейцина также не увеличивали прирост мышечной силы и массы после 12-недельной программы силовых тренировок у молодых мужчин, тренировавшихся с отягощениями и потреблявших достаточное количество белка с рационом.

Глюкозамин

Глюкозамин в организме выступает в качестве одного из "строительных блоков" хрящей [44]. A. Eraslan и B. Ulkar [26] начинали пероральное введение либо глюкозамина сульфата, либо плацебо в конце 6-й недели послеоперационного периода, когда была достигнута полная весовая нагрузка. Для оценки динамических функций коленного сустава в конце 16-й недели после операции проводили изокинетические тесты с концентрическими сокращениями по значениям "пикового крутящего момента" и "средней мощности". Субъекты получали один и тот же протокол реабилитации под наблюдением одного и того же физиотерапевта. Авторы не обнаружили статистически значимых различий показателей мышечной силы по прошествии 8 нед приема между группами обследованных. Это согласуется с результатами РКИ M.N. Babur и соавт. [45] с участием 2 групп, принимавших глюкозамин и натриевую соль хондроитина сульфата или плацебо в течение 4 нед. Не наблюдалось существенных различий по показателям изометрической силы ни через 2, ни через 4 нед после вмешательства между 2 группами (p>0,05).

Креатин

Содержащие креатина моногидрат БАД считаются безопасным выбором и наиболее эффективной эргогенной БАД для спортсменов [12]. Кроме того, было показано, что прием креатина полезен для неортопедических групп населения, улучшая восстановление и помогая предотвратить травмы [18]. Поскольку сообщалось, что БАД с креатином способствуют увеличению мышечной массы и увеличению силы, возник интерес к изучению влияния креатина на скорость атрофии мышц в результате иммобилизации конечностей и/или во время реабилитации [13, 46]. T.F. Tyler и соавт. [27] измеряли силу сгибателей бедра, отводящих и приводящих мышц с помощью ручного динамометра до операции и через 6, 12 нед или 6 мес после операции. Моменты разгибания и сгибания колена измерялись изокинетически при 60 и 180°/с в положении сидя. После субмаксимальной разминки выполняли 5 максимальных сокращений со скоростью 60°/с, затем - 20 максимальных сокращений со скоростью 180°/с. Мышечную силу определяли как пиковый крутящий момент при 60°/с, а мышечную мощность - как ее средний показатель во время 20 сокращений при 180°/с. В период от 6 до 12 нед наблюдалось значительное увеличение силы на пораженной стороне при разгибании колена (47%), сгибании колена (27%) и бедра (20%), отведении (9%) и приведении бедра (17%) (р<0,001), а также было выявлено значительное увеличение мощности пораженной конечности при разгибании (46%) и сгибании колена (26%) (р<0,0001). Но эти эффекты не были связаны с приемом креатина (p=0,11, p=0,82).

В свою очередь, систематический обзор и метаанализ E.E.P. Dos Santos и соавт. [47] показал, что добавки креатина в сочетании с тренировками с отягощением в небольшой когорте пожилых женщин увеличили мышечную силу, когда продолжительность вмешательства составляла не менее 24 нед, однако не оказали никакого влияния на мышечную массу.

Заключение

Это первый систематический обзор данных о влиянии БАД на мышечную массу и силу оперированной конечности после реконструкции ПКС. Небольшое количество включенных исследований являлось ограничением для этого систематического обзора. Большинство представленных в данном обзоре исследований имели умеренный или высокий риск систематической ошибки. В исследованиях часто наблюдалась неоднородность: тип БАД, способ ее приема, дозировка и представленные результаты. Ни одна из используемых БАД не оказала статистически значимого влияния на силу мышц оперированной конечности после реконструкции ПКС. Что касается гипертрофии мышц бедра, то единственное значительное улучшение было связано с приемом лейцина и увеличением окружности бедра на расстоянии 10 см от надколенника в основной группе. Можно предположить, что причиной резистентности мышечной ткани на прием хорошо себя зарекомендовавших БАД является сопротивление синтеза мышечного белка к любым анаболическим, пищевым стимуляциям. Возможные механизмы сопротивления синтеза мышечного белка: 1) с гиподинамией нарушается переваривание белков, всасывание аминокислот и других биологически активных веществ; 2) изменение микрососудов вокруг травмы нарушает перфузию и поглощение аминокислот и биологически активных веществ мышцами; 3) нарушается сигнализация анаболических путей.

Таким образом, результаты этого исследования говорят о том, что, пока не восстановится трофика мышечной ткани, пероральный прием БАД не оказывает положительного эффекта при реабилитации пациентов после реконструкции ПКС. Однако при интерпретации результатов данного обзора следует проявлять определенную осторожность, учитывая небольшое количество исследований и различия в их характеристиках. Будущие исследования должны оценить эти же добавки и изучаемые переменные с большим числом участников, принимая во внимание влияние физических упражнений на параметры реабилитации. Необходимы дальнейшие хорошо спланированные высококачественные плацебо РКИ, чтобы установить клиническую эффективность применения БАД при реабилитации пациентов после реконструкции ПКС.
