Features of the metabolism of polyunsaturated fatty acids in type 2 diabetes mellitus

Abstract

Type 2 diabetes mellitus (T2DM) is a widespread disease with a high risk of cardiovascular complications, disability and mortality. The progression of T2DM is closely related to lipid metabolism disorders, caused both by insufficient intake of polyunsaturated fatty acids (PUFAs), and by a violation of their endogenous metabolism. Desaturase enzymes, FADS1/2, are involved in the regulation of PUFA metabolism. Violation of the functioning of FADS1/2 and their genes leads to a change in the biosynthesis of PUFAs and the fatty acid composition of cell membranes.

The purpose of this research was to summarize the data of modern literature on the metabolism of PUFAs and the effect of FADS genetic variants on the fatty acid composition of cell membranes in T2DM.

Material and methods. The search and analysis of publications was carried out using the PubMed, MEDLINE, Web of Science databases, mainly for the last 10 years, using the search keywords: polyunsaturated fatty acids, fatty acid desaturases, desaturase genes, type 2 diabetes mellitus.

Results. In the pathogenesis of T2DM and its complications, many factors play a role, including impaired PUFA metabolism. A large evidence base has been accumulated on the effect of PUFAs on cardiometabolic risk factors in T2DM. The effect of desaturase activity on the fatty acid composition of cells was identified as the most important link in the metabolism of PUFAs. Focusing on the modulation of desaturase activity and studying the polymorphism of fatty acid desaturase genes may be a useful therapeutic option in the treatment of patients with T2DM and its complications.

Conclusion. A promising direction of scientific research in the treatment and prevention of T2DM and its complications is the study of genetic mechanisms associated with the metabolism of PUFAs and their metabolites.

В работе S. Sergeant и соавт. [24] изучали врожденную способность человека синтезировать ПНЖК и активность FADS у американцев африканского и европейского происхождения с СД2/метаболическим синдромом. Американцы африканского происхождения продемонстрировали заметно более высокие уровни арахидоновой кислоты в сыворотке крови (9,8±1,9%) от общего количества жирных кислот по сравнению с этим показателем у американцев европейского происхождения (7,9±2,1%). При этом активность FADS1 составила 6,9±2,2 у американцев африканского происхождения и 5,4±2,2 у американцев европейского происхождения. 7 однонуклеотидных полиморфизмов локуса FADS показали корреляцию с уровнем арахидоновой, эйкозапентаеновой и дигомо-γ-линоленовой кислот у американцев европейского происхождения. Американцы европейского происхождения, гомозиготные по минорному аллелю (T), имели значительно более низкие уровни арахидоновой кислоты, но более высокие уровни дигомо-γ-линоленовой кислоты по сравнению с гомозиготами по основному аллелю (GG). Эти данные говорят о том, что существуют важные различия в способности синтезировать длинноцепочечные ПНЖК, что можно объяснить генетическими механизмами в разных популяциях.

Целью исследования V. Mansouri и соавт. [25] было установление связи между полиморфизмами rs174537 в гене десатуразы жирных кислот FADS1 и rs174575 в гене FADS2 у лиц с СД2, проживающих в Иране. Результаты показали, что среди 50 участников с СД2 68% пациентов имели мутантный аллель rs174537 гена FADS1 и 26% - rs174575 гена FADS2. Лица, носители мутантного аллеля rs174537, более склонны к развитию СД2, вместе с тем у этих лиц выявлены более благоприятные показатели липидного профиля (общий холестерин и холестерин липопротеинов низкой плотности). Лица, имеющие мутантный аллель rs174575, менее склонны к развитию СД2, но имеют более высокий уровень триглицеридов и индекс массы тела/ожирение. Исследователи пришли к выводу, что выявление полиморфизмов генов FADS1 и FADS2 может быть использовано для прогнозирования предрасположенности к кардиометаболическим заболеваниям. Линолевая и α-линоленовая кислоты преобразуются в арахидоновую кислоту с помощью Δ5-десатуразы и Δ6-десатуразы. Десатуразы, регулирующие превращения жирных кислот, влияют на производство молекул, модулирующих воспалительные реакции: простагландины, тромбоксаны, лейкотриены, липоксин, а также регулируют работу рецепторного аппарата, от этих процессов зависит скорость развития СД2.

J. Kroger и соавт. показали, что активность Δ5-десатуразы обратно, а Δ6-десатуразы напрямую связана с риском развития СД2 [26]. При этом влияние десатуразной активности на метаболизм глюкозы и инсулинорезистентность определяется полиморфизмом генов FADS1 и FADS2.

Изменения активности десатуразы, жирнокислотного состава клеток, уровня медиаторов воспаления, компонентов метаболического синдром дают возможность предотвратить/отсрочить развитие СД2. Анализируя изменения генетического разнообразия десатураз, можно вычленить 2 возможных пути, приводящих к развитию СД2: развитие инсулинорезистентности, как показано в исследовании М.-С Huang и соавт. [23], и влияние генетических полиморфизмов на медиаторы воспаления - метаболиты арахидоновой кислоты [27].

Целью исследования S.-W. Li и соавт. [28] было выявление возможной связи между жирнокислотным составом клеток и полиморфизмами генов десатураз жирных кислот (FADS1 и FADS2) и развитием СД2 и ИБС. В исследование вошли 234 пациента с СД2, 200 пациентов с ИБС, 185 пациентов с СД2 и ИБС и 253 здоровых людей. У пациентов с СД2 и ИБС был выявлен самый высокий уровень арахидоновой кислоты в плазме, дигомо-γ-линоленовой кислоты и активности Δ6-десатуразы и низкий уровень стеариновой, линоленовой кислот и НЖК. Уровень плазменных эйкозапентаеновой и докозагексаеновой кислот был повышен у пациентов с СД2, но значительно снижен у пациентов с ИБС. Кроме того, пациенты с СД2 с генотипом rs174537 GG имели риск развития ИБС [отношение шансов 1,763; 95% ДИ 1,143-2,718; p=0,010] при повышенном уровне холестерина липопротеинов низкой плотности, арахидоновой кислоты и активности Δ6-десатуразы в плазме. Результаты показали, что полиморфизм FADS (в частности, rs174537) связан с жирнокислотным составом плазмы и активностью десатураз у пациентов как с СД2, так и с ИБС, что может повышать риск ИБС у пациентов с СД2.

Согласно данным Национального центра биотехнологической информации (National Center for Biotechnology Information, США), частота встречаемости минорных аллелей наиболее изученных генетических полиморфизмов, регулирующих активность десатураз, составляет для rs174547 (ген FADS1) 32,0% в европейских популяциях и 46,0% в азиатских популяциях; rs174583 (ген FADS2) - 34,0% в европейских популяциях и 47,0% - в азиатских [29, 30]. В Российской Федерации этот вопрос практически не изучен, особенно относительно влияния этих полиморфизмов на риск развития СД2.

Заключение

На сегодняшний день можно с уверенностью сказать, что несомненным достижением нутрициологии последних лет являются результаты детального изучения роли ПНЖК в обеспечении нормального развития и адаптации организма человека к неблагоприятным факторам окружающей среды, профилактики и лечения заболеваний внутренних органов [31-35]. В нашей стране вопросами взаимосвязи ПНЖК с различными физиологическими и патологическими состояниями человека активно занимался профессор М.М. Левачев [36].

Влияние активности десатураз на жирнокислотный состав клеток было идентифицировано как существенный фактор риска развития СД2 и ИБС. На уровни ПНЖК в крови и тканях влияет не только недостаточное алиментарное поступление, но и в значительной степени генетическая наследуемость, в частности, в регуляции метаболизма жирных кислот важное значение имеет активность десатураз. Δ5- и Δ6-десатуразы, кодируемые соответственно генами FADS1 и FADS2, являются ферментами, ограничивающими скорость превращения ПНЖК. Изменения активности этих форм десатураз связаны с рядом заболеваний, в том числе СД2 и ИБС. Сосредоточение внимания на модуляции активности десатураз может быть полезной терапевтической опцией при лечении кардиометаболических заболеваний.

Недостаточная частота выявления пациентов с СД2 представляет проблему для здравоохранения, поскольку значимая часть пациентов остается недиагностированной, не получает лечения и имеет высокий риск развития ССЗ.

Изучение генетических полиморфизмов, ассоциированных с показателями метаболизма жирных кислот, жирнокислотным составом клеточных мембран, может явиться новой опцией для раннего выявления пациентов с высоким кардиометаболическим риском. Появление новых возможностей для скрининга пациентов с СД2 и его сердечно-сосудистых осложнений открывает перспективы для ранней диагностики и разработки новых терапевтических подходов при комплексном лечении этого заболевания.
