Vitamin D and childhood disability: diseases of the musculoskeletal system and connective tissue in the Khanty-Mansi Autonomous Okrug – Ugra

Abstract

Vitamin D deficiency is more prevalent among children with musculoskeletal and connective tissue disorders (MSCTD), which plays a significant role in childhood disability, which ranks sixth in the structure of childhood disability in the Russian Federation.

The aim of the research was to study of the relationship between the incidence of childhood disability associated with MSCTD and the state of vitamin D status of the population living in the territory of the Khanty-Mansiysk Autonomous Okrug – Ugra.

Material and methods. A correlation analysis was carried out between the level of childhood disability (that arose in connection with MSCTD in children aged 0–17 years in the Khanty-Mansi Autonomous Okrug – Ugra in 2021) and the prevalence of severe vitamin D deficit [serum 25(OH)D <10 ng/ml] among the population (12 city and 5 district municipalities), using the INVITRO-Ural LLC database (31 595 anonymized measurements of vitamin D level in Ugra residents). In addition, a correlation analysis was conducted between the total incidence of certain types of MSCTD in the constituent entities of the Russian Federation and the geographical latitude of the administrative center of the subjects of the Federation.

Results. In the Khanty-Mansi Autonomous Okrug – Ugra, the frequency of childhood disability resulting from MSCTD is associated with a statistically stable (p=0.01) directly proportional relationship with the prevalence of severe vitamin D deficit in the residents of the municipality territory. In the Russian Federation, the dependence of the general morbidity of MSCTD in children (arthropathy, juvenile arthritis, and damage to the tendon synovial membranes) is very stable (p<0.0001) directly proportional associated with the geographical latitude of the territory. This indicates the impact of reduced levels of ultraviolet radiation and, accordingly, the average blood level of vitamin D in the population high in latitudes, along with other reasons, on human health.

Conclusion. Low levels of vitamin D have a negative impact on the activity of MSCTD in children and the associated disability. To justify the recommended daily dose and duration of vitamin D intake, which in some cases can reduce the activity of MSCTD, it is necessary to measure its initial level in the blood serum of patients at risk.

Согласованное заявление Второй международной конференции по спорным моментам в отношении витамина D гласит, что прием в суточной дозе 4000 МЕ является безопасным верхним уровнем потребления витамина D [17]. При этом краткосрочное применение более высоких доз (до 10 000 МЕ/сут), вероятно, не будет сопровождаться осложнениями и не приведет к повышению уровня 25(OH)D в сыворотке крови выше 100 нг/мл. Применение даже достаточно высокой дозировки витамина D (4000-10 000 МЕ/сут) при некоторых заболеваниях может быть недостаточным для лечения пациентов, которым требуются значительно более высокие суточные дозы витамина D. Рекомендованная дозировка также может оказаться недостаточной для лиц с выраженным дефицитом витамина D, которым могут быть показания к быстрому повышению уровня путем использования более высоких доз в краткосрочной перспективе [17]. Разумеется, для того чтобы дать обоснованные рекомендации по терапевтическому применению препаратов витамина D, необходим мониторинг его начального уровня [14, 18-20].

Концентрация витамина D в сыворотке крови человека напрямую зависит от воздействия ультрафиолетового излучения солнца. Несколько факторов окружающей среды, таких как широта, высота над уровнем моря, время года и преобладающие погодные условия, определяют, доступен ли солнечный свет достаточной силы, чтобы стимулировать превращение 7-дегидрохолестерина в коже в холекальциферол. Это приводит к сезонным колебаниям уровня 25(OH)D и эффекту, основанному на географическом местоположении, в котором проживает человек (расстояние от экватора и высота над уровнем моря) [22]. Поскольку до 80-90% выработки холекальциферола (витамина D3) в эпидермисе человека приходится на долю ультрафиолетового излучения в диапазоне 290-315 нм [21], в высоких широтах чаще отмечается пониженный уровень и более значительная распространенность дефицита и глубокого дефицита витамина D.

Так, у жителей юга Англии (по сравнению с жителями северных территорий) наблюдается более низкий риск дефицита витамина D (оценка риска 0,78), даже после корректировки на социально-экономические и традиционные предикторы (ожирение, возраст, образ жизни и т.д.) [23].

В масштабном исследовании Д.В. Смирновой и соавт. использованы данные о лицах, проходивших тестирование на концентрацию витамина D в сыворотке крови с 2013 по 2018 г. в лабораториях ООО "ИНВИТРО" в Российской Федерации (n=30 040, возраст старше 18 лет). Наблюдалась нелинейная зависимость обеспеченности витамином D от широты. Самые высокие концентрации 25(OH)D отмечались вблизи 54 °с. ш., снижаясь в более южных широтах для женщин (в том числе Северный Кавказ) и в более северных широтах для обоих полов. У мужчин совершенно очевидна тенденция роста распространенности дефицита и глубокого дефицита в более высоких широтах [24].

Полученные нами закономерности, проиллюстрированные рис. 3-5, согласуются с приведенными выше результатами исследований зарубежных и отечественных ученых и говорят о том, что население северных территорий имеет в среднем более низкий уровень витамина D в сыворотке крови, что сопровождается ростом уязвимости к заболеванию КМС и СТ.

Заключение

Результаты проведенного исследования свидетельствуют о том, что в муниципальных образованиях ХМАО-Югры повышение распространенности глубокого дефицита витамина D сопровождается ростом детской инвалидности, связанной с заболеваниями КМС и СТ. В Российской Федерации в целом отмечается тенденция роста общей заболеваемости КМС и СТ среди детей и подростков, проживающих в более высоких широтах, что наряду с другими причинами также связано со снижением влияния ультрафиолетового излучения и выработкой меньшего количества витамина D в коже человека.

В связи с этим проблеме профилактики заболеваемости КМС и СТ среди детей и подростков, особенно острой для населения северных территорий, следует уделять более пристальное внимание как работникам медицинских учреждений, так и сотрудникам органов здравоохранения всех уровней - от муниципального до федерального. Решение этой проблемы возможно только при условии своевременного мониторинга обеспеченности витамином D, и при обнаружении дефицита и/или недостаточной концентрации кальцидиола в сыворотке крови пациентов - приема витамина D для поддержания концентрации 25(OH)D на достаточном уровне (≥30 нг/мл).
