Effect of carnosine and α-lipoic acid on hepatocyte apoptosis and the cytokine profile in induced fatty liver disease in Wistar rats
AbstractNon-alcoholic fatty liver disease (NAFLD) is now a common liver disease affecting about a third of the world’s population. In this regard, the issue of studying the pathogenetic factors of the development of this disease in order to select adequate drug therapy and biologically active substances with antioxidant properties regulating the balance of pro- and anti-inflammatory cytokines is of particular relevance.
The aim of the study was to assess the effect of minor biologically active substances - carnosine and α-lipoic acid on hepatocyte apoptosis and the cytokine profile in the experimental model of the initial stage of NAFLD.
Material and methods. The studies were performed on male Wistar rats with initial body weight of 150±10 g. Animals were divided into 5 groups of 8 rats each. Within 8 weeks, rats of the 1st group (control) received a complete modified diet AIN93M, in which soybean oil was replaced with sunflower oil and lard (1:1). Rats of the experimental groups consumed high-calorie choline-deficient diet (HCCDD), in which fat content was 45%, fructose content - 20% of the energy value of the diet. Rats of the 2nd group were fed HCCDD without any supplements, the 3rd group - with the addition of carnosine (75 mg/kg body weight), the 4th group - with the addition of α-lipoic acid (75 mg/kg body weight), the 5th group - with the addition of carnosine and α-lipoic acid in a total dose of 150 mg/kg body weight. The study of rat hepatocyte apoptosis was performed by flow cytometry. Hepatocytes were stained with annexin V and vital dye 7-aminoactinomycin, followed by detection on an flow cytometer. The content of cytokines and chemokines (IL-1α, IL-10, IL-17А, M-CSF, MIP-1α, MIP-3α, RANTES) in the cytoplasmic fraction of liver tissue was determined by multiplex immunoassay.
Results and discussion. On the model of the initial stage of development of NAFLD in male Wistar rats the еnrichment of HCCDD with carnosine and α-lipoic acid had demonstrated a protective effect on hepatocytes with a decrease in apoptosis intensity to the level in control rats. Under the influence of HCCDD, an increase in the content of M-CSF and MIP-1α and a decrease in the levels of MIP-3α and RANTES, stimulating the migration and differentiation of various immunoregulatory populations to the parenchyma at an early stage of the formation of fatty hepatosis, in the cytoplasmic fraction of liver tissue were detected. Moreover, a decrease in the level of proinflammatory cytokines IL-17A and IL-1α and an increase in IL-10 produced mainly by Treg-populations indicate the absence of pronounced inflammatory changes in the liver of male Wistar rats at the initial stages of development of fatty dystrophy.
Conclusion. Enrichment of HCCDD with both carnosine and а-lipoic acid in Wistar rats had a protective effect on hepatocytes with a decrease in apoptosis to a level in control rats. The increase in the IL-10/IL-17A ratio indicates the activation of anti-inflammatory mechanisms due to the functional predominance of Treg-cells over Th1/Th17 lymphocytes.

Печень, состоящая на 20% из непаренхиматозных клеток, кроме обеспечения метаболического гомеостаза, участвует в регуляции иммунитета и иммунной толерантности. В избыточном накоплении жировых включений и формировании гепатоза с апоптотическими изменениями паренхимы, особенно при потреблении животными ВКХДР (см. таблицу), ведущая роль принадлежит синусоидальным эндотелиальным клеткам (HSC). Кроме того, в этих процессах участвуют дендритные клетки, макрофаги и мигрирующие из сосудистого русла гранулоциты, моноциты и лимфоидные популяции [26, 27]. С этим согласуется обнаруженное среди хе-мокинов в печени статистически значимое возрастание содержания M-CSF при потреблении крысами ВКХДР и уровня MIP-1a при всех опытных рационах по сравнению с контролем (см. рис. 1). Напротив, содержание хемоаттрактантов для лимфоидных популяций MIP-3α и RANTES может не увеличиваться, а снижаться на начальных этапах жирового гепатоза (см. рис. 1) [28].
Активация и способность синусоидальных скоплений HSC к росту и накоплению липидов не зависит ни от исследуемой модели, ни от характера альтерирующего воздействия (пищевое, токсическое и т.п.). Установлено, что этот феномен гипертрофии тесно связан с продукцией коллагена и степенью выраженности фиброза в печени [29, 30].
При развитии жирового гепатоза в HSC активируется экспрессия большого числа генов (в частности, Ki-67, CXCL14, семейство ингибиторов апоптоза - IAP, холестерин 25-гидроксилаза и многих др.), регулирующих процессы пролиферации и играющих защитную роль в выживаемости непаренхиматозных клеток печени при развитии фиброзных изменений [26].
Активация, или супрессия, генов хемокинов CCL3 (MIP-1α), CCL4 (MIP-3α), CLL5 (RANTES), изменение содержания которых обнаружено в нашем исследовании (см. рис. 1, 2), а также активация семейства лигандов CXCL14 выступают при жировой дистрофии важнейшими сигналами для мобилизации иммунных клеток в очаги воспаления и формирующейся фиброзно-дистрофической трансформации паренхимы печени [28]. Кроме того, позитивная индукция сигнальных путей: холестерин 25-гидроксилаза (CH25H), транскрипционные факторы (sterol regulatory element-binding protein) SREBP-1 и SREBP-2 -способствует приобретению HSC схожего с адипоцитами фенотипа и избыточному накоплению липидов [29, 30].
Среди интерлейкинов в печени крыс 2-й группы выявлено значимое (р<0,05) по сравнению с контрольной группой снижение уровня провоспалительных цитокинов IL-17А и IL-1α и прогрессивный рост продуцируемого преимущественно Treg-популяциями IL-10 (см. рис. 2). Это можно расценить как косвенные признаки отсутствия явных воспалительных изменений в печени на начальном этапе жировой дистрофии. На ранних этапах жировой трансформации печеночные дендритные клетки (DCs), распознав присутствующие в очагах воспаления антигены (процесс, как и активация непаренхиматозных клеток, неспецифичен), активируют цитокиновые сети позитивной дифференцировки и миграции Treg за счет нарастания продукции и увеличения общего и тканевого (преимущественно в селезенке) содержания IL-10. При дистрофических изменениях в печени (независимо от этиологического фактора) неизбежно происходят морфофункциональные изменения в селезенке [25, 31, 32]. В результате нарастает содержание в печени IL-10 и соотношение IL-10/IL-17A за счет активации Treg-популяций на фоне как ВКХДР, так и добавления антиоксидантов. Дисрегуляция и блокада этих механизмов при прогрессировании инсулинорезистентности и ожирении приводят к формированию очагов фиброза, повышению выброса провоспалительных факторов, метаболическим нарушениям и печеночной дисфункции [27].
Снижение содержания цитокинов IL-17А и IL-1α в печени крыс 3-й и 4-й групп по сравнению с крысами 2-й группы и контрольной, увеличение соотношения IL-10/ IL-17A свидетельствует о том, что используемые в рационах биологически активные вещества: карнозин и α-липоевая кислота - способны уменьшать воспалительные изменения при наблюдаемой жировой дистрофии печени на фоне ВКХДР. Однако механизмы положительного влияния этих добавок различаются и не до конца изучены. В частности, α-липоевая кислота - природный эндогенный антиоксидант, действует как агонист PPAR-γ, уменьшая проявления окислительного стресса. Предполагается, что α-липоевая кислота может оказывать метаболический и иммуномодулирующий эффект при воспалительных процессах различного генеза.
Было показано, что α-липоевая кислота значимо снижает процентное содержание Th1 и Th17. Напротив, наблюдается стимуляция дифференцировки и созревания Treg-популяций в селезенке и системная индукция PPAR-γ сигнальных путей с ингибированием воспалительных реакций [33]. При этом на рис. 1 показано, что добавление в рацион α-липоевой кислоты вызывает тенденцию к росту хемоаттрактанта для лимфоцитов RANTES по сравнению с крысами, потреблявшими ВКХДР На моделях с депрессией генов PPAR-γ/α или блокадой PPAR в печени, напротив, наблюдаются избыточное накопление липидов с развитием жирового гепатоза и воспалительная трансформация паренхимы [34].
Карнозин, как и а-липоевая кислота, является природным антиоксидантом и обладает способностью улучшать метаболические процессы на моделях диабета у животных. Механизмы этого влияния до сих пор не полностью расшифрованы. Было показано, что карнозин снижает уровень циркулирующего связывающего инсулиноподобного фактора роста 1 (IGFBP1) и экспрессию этого белка в печени за счет прямой супрессии индуцируемого гипоксией фактора-1α (HIF-1α) -центрального медиатора, индуцирующего продукцию IGFBP1 на фоне гипоксии. С другой стороны, карнозин повышает уровень циркулирующего инсулина, и за счет снижения содержания глюкозы возрастает его концентрация в плазме [35]. Уровень IGFBP1 увеличивается системно и в печени при воспалительных процессах, окислительном стрессе и ожирении. Этот белок вовлечен в регуляцию процессов апоптоза в печени. Инсулин, уровень которого возрастает при добавлении карнозина, прямо супрессирует промоутеры IGFBP1 - протеинкиназу B (Akt) и транскрипционный фактор FoxO1 (Forkhead box O1). Медиаторы воспаления: TNF-α, IL-1α, IL-1β и IL-6 - способны индуцировать экспрессию мРНК и продукцию IGFBP1 in vitro. Семейство IL-1 - основные индукторы биосинтеза IGFBP1 при воспалении [36]. Снижение содержания I L-1α (р<0,05 в сравнении с контролем и со 2-й группой) и IL-17A (р<0,05 с контролем, р<0,1 со 2-й группой) за счет потребления животными карнозина можно расценить как инсулинонезависимый путь редукции уровня IGFBP1 в механизмах супрессии воспаления, уменьшения количества клеток в апоптозе и улучшения метаболических процессов при жировом гепатозе. Однако стоит отметить, что при сочетанном применении карнозина и α-липоевой кислоты в дозе 150 мг на 1 кг массы тела не удалось достичь суммирования противовоспалительных эффектов, наблюдаемых при раздельном использовании данных биологически активных веществ.
Под влиянием ВКХДР у крыс 2-й группы в цитоплазматической фракции ткани печени обнаружено повышение содержания M-CSF и MIP-1α и снижение уровней MIP-3α и RANTES, стимулирующих миграцию и дифференцировку различных иммунорегуляторных популяций в паренхиму на раннем этапе формирования жирового гепатоза. При этом снижение уровня провоспалительных цитокинов IL-17A и IL-1α и увеличение содержания IL-10 свидетельствуют об отсутствии выраженных воспалительных изменений в печени самцов крыс линии Вистар на начальных этапах развития жировой дистрофии.
Заключение
Обогащение ВКХДР как карнозином, так и α-липоевой кислотой у крыс линии Вистар оказало протективный эффект на гепатоциты со снижением интенсивности апоптоза до уровня у крыс контрольной группы. Рост соотношения IL-10/IL-17A свидетельствует об активации противовоспалительных механизмов за счет функционального преобладания Treg-клеток над лимфоцитами Th1/Th17.
Апоптоз гепатоцитов при алиментарно-индуцированной дистрофии печени является определяющим фактором развития прогрессирующих форм неалкогольного жирового гепатоза. В связи с этим особую актуальность приобретает вопрос выбора адекватной лекарственной терапии и применения биологически активных веществ, обладающих антиоксидантными и регулирующими баланс про- и противовоспалительных цитокинов свойствами.
