Gliadomorphin, casomorphin, and intestinal fatty acid binding protein in children with autism spectrum disorders
AbstractDiet therapy for autism spectrum disorders (ASD) remains one of the most popular alternative therapies. despite conflicting opinions regarding the effectiveness of the dietary approach. According to the theory of exorphin intoxication, gluten and casein peptides enter the bloodstream through the mucous membrane of the small intestine, penetrate the blood-brain barrier and affect the neurons of the cerebral cortex. The well-known hypothesis of the relationship between autism and gluten intolerance is based on this theory.
The aim of this work was to study the correlation between the blood concentration of intestinal fatty acid - binding protein (I-FABP) and gliadomorphin and casomorphin as markers of opioid intoxication, depending on the use of diet therapy in children with ASD.
Material and methods. The study included 85 patients aged 3 to 15 years with an established diagnosis of ASD. The first group consisted of 36 children who followed a gluten-free diet (GFD) for at least 6 months, 3 of them also followed a casein-free diet (CFD), the second group included 49 patients with ASD who had no dietary restrictions. The concentration of I-FABP, gliadomorphin, and casomorphin in the blood serum was determined by enzyme immunoassay in all patients.
Results. In children with ASD who followed GDD, the average values of the studied parameters were significantly lower than in patients with ASD who have no dietary restrictions: gliadomorphine - 0.98±1.27 vs 1.68±0.97 ng/ml, casomorphine - 1.62± 0.76 vs 2.37±0.53 pg/ml, I-FABP - 156.2±102.16 vs 528.26±255.95 pg/ml (p<0.01). In patients with ASD using diet therapy, there was a significant increase in gliadomorifin (r=0.64, p=0.0001) and casomorphin (r=0.53, p=0.001) with an increase in I-FABP. In children with ASD, not adhering GFD, there was also an increase in blood gliadomorphin (r=0.30, p=0.036) with an increase in I-FABP level; this trend was not observed relative to casomorphin (r=-0.0050, p=0.973). Perhaps, with the expansion of the sample, this pattern will also be observed in children who are on a regular diet.
Conclusion. When including diet therapy in the therapeutic treatment of autism, it is necessary to take into account the individual intolerance to gluten and casein, conduct additional examinations in order to specify the nature of the intolerance and the need to prescribe a diet.

Состояние кишечника у пациентов с РАС привлекает внимание ученых как возможный фактор развития этой патологии. Со стороны толстой кишки речь идет о нарушении микробиома, который в настоящее время активно изучается и уже получены интересные данные об особенностях состава и роли микробиоты у детей с аутизмом. Состояние тонкой кишки изучается прежде всего с позиции повышенной проницаемости, в частности для глютена и казеина. Изменение кишечной проницаемости по результатам теста с лактулозой/маннитом было выявлено у 36,7% пациентов с РАС и у 21,2% их родственников, в то время как среди здоровых лиц из группы контроля - лишь у 4,8% [23]. При этом пациенты, которые придерживались диетотерапии, имели более низкие показатели проницаемости, чем обследуемые с РАС без ограничений в питании. X. Wang и соавт. в 2019 г. определили наличие повышения концентрации I-FABP (и, соответственно, повышения проницаемости кишечника) при болезни Альцгеймера, что косвенно обосновывает теорию изменения состояния энтероцитов при заболеваниях нервной системы [24]. Полученные нами данные в отношении статистически значимого повышения I-FABP у детей с РАС, не имеющих ограничений в питании, могут служить основанием для подтверждения повреждения кишечника при РАС и экзорфиновой интоксикации при потреблении глютена и казеина и, как следствие, повышения проницаемости слизистой оболочки тонкой кишки. В результате нашего пилотного исследования впервые получены данные о статистически значимо более высоком содержании I-FABP у детей с РАС, не имеющих ограничений в питании, по сравнению с теми, кто находится на диете, а также выявлена средняя корреляционная зависимость между уровнем I-FABP и концентрацией глиадо- и казоморфинов. Это, во-первых, подтверждает гипотезу развития экзорфиновой интоксикации при РАС, а во-вторых, указывает на наличие и значение изменений в слизистой оболочке тонкой кишки, характер которых еще требует уточнения в дальнейшей работе. Это может быть повреждение энтероцитов при воздействии разных факторов, воспаление и в итоге повышенная проницаемость.
Таким образом, на сегодняшний день нельзя недооценивать значение опиоидных пептидов (глиадо- и казоморфина) при РАС, изучение механизмов влияния которых представляется актуальным для выявления показаний к использованию диетотерапии у детей при РАС и определения объективных диагностических маркеров непереносимости глютена и казеина у больных. Проведенное нами исследование, несмотря на ряд ограничений, позволило получить предварительные данные, указывающие на роль глиадо- и казоморфина при РАС и значение диетотерапии.
Выводы
1. У детей с РАС, соблюдающих БГД, средняя концентрация опиоидных пептидов была статистически значимо ниже, чем у пациентов с РАС, не имеющих ограничений в питании (p≤0,01).
2. У пациентов с РАС, использующих диетотерапию, отмечено достоверное нарастание уровня глиадоморфина (r=0,64, p=0,0001) и казоморфина (r=0,53, р=0,001) при увеличении I-FABP У детей с РАС, не приверженных к соблюдению БГД, также отмечается повышение глиадоморфина в крови (r=0,30, р=0,036) при повышении уровня I-FABP, относительно казоморфина такой тенденции не отмечено.
3. При включении диетотерапии в терапевтический курс при аутизме необходимо учитывать индивидуальную непереносимость глютена и казеина, проводить дополнительные обследования с целью конкретизации характера непереносимости и необходимости назначения диеты.
