Features of the nutritional status and nutrition of children with autistic disorders

Abstract

In recent years, there has been a steady increase in the number of children with autism spectrum disorders (ASD). According to the Ministry of Health of Russia, the incidence of ASD in children under 2 years of age is 5:10 000, at the age of 4 years - 18:10 000. ASD is a complex multisystem disease that affects metabolic and neurobiologicalprocesses. The aim of the research was to analyze the dietary habits of children with autism spectrum disorders.

In ASD, physical developmental disorders occur, both in the direction of increasing and decreasing body weight, which is accompanied by pathology of the gastrointestinal tract (GIT) and eating disorders. Diseases of the digestive system are associated with both inflammatory and functional disorders in the GIT, changes in the immune status, autonomic tone of the nervous system and the composition of the GIT microbiota. Eating disorders in children with ASD lead to negative changes in the child’s nutritional status and aggravates the course of gastrointestinal diseases. Nutritional status in children with ASD is characterized by an excess of calories in the diet (or a sharp decrease in it), excessive consumption of fat, sugar and salt, deficiency of vitamins, carotenoids, mineral substances (calcium in 45.1%, lithium in 30-35%, potassium in 70% of children with ASD). It has been found that nutritional deficiencies in ASD exacerbate neuropsychiatric symptoms. Lack of vitamins leads to metabolic disorders, delayed physical and mental development, rapid fatigue, endocrine dysfunction, and aggravation of the symptoms of ASD.

Conclusion. Nutritional adjustments in children with ASD can lead to an improvement in non-verbal IQ and a decrease in the clinical manifestations of autism. The issue of developing recommendations for the organization of nutrition for children with ASD in educational institutions remains highly relevant.

По сведениям немногочисленных публикаций, наряду с дефицитом витаминов для пациентов с аутизмом характерен дефицит минеральных веществ. Так, дефицит железа зарегистрирован у 37,3% пациентов с РАС и является одним из самых частых алиментарных нарушений детского возраста не только у лиц с аутизмом, но и в популяции в целом [53]. В детском возрасте дефицит этого микроэлемента может оказывать значительное негативное влияние на развитие и состояние ребенка. По данным многих наблюдений, в условиях недостаточности железа регистрируются более низкие индексы психомоторного развития, причем существует мнение, что задержка психомоторного развития носит необратимый характер, несмотря на проведенное впоследствии лечение [53]. Дефицит йода выявляется у 70-80% детей с РАС (на 45% чаще по сравнению с детьми без отклонений в показателях здоровья) [54], вероятная причина - дисфункция щитовидной железы, диагностируемая у многих пациентов с РАС. Йод необходим для нормального созревания нервной системы, умственного и физического развития, а его недостаточность зачастую приводит к тяжелым неврологическим нарушениям. В той же мере на развитие организма оказывает свое влияние цинк, который входит в состав многих ферментов и принимает активное участие в регуляции процессов роста и дифференцировки клеток, в том числе ЦНС. Недостаточность цинка зарегистрирована у 39% детей с аутизмом [13].

Помимо дефицита эссенциальных микроэлементов, у пациентов с аутизмом отмечена недостаточность и других минеральных веществ в крови: кальция - у 45,1%, лития - у 30-35%, а также калия у детей с мышечной дистонией (около 70% детей РАС) [12, 53-55].

Таким образом, недостаток нутриентов - одна из серьезных проблем детского населения, при этом для детей с РАС характерны более выраженные изменения в нутритивном статусе [14].

Необходимо подчеркнуть, что вопрос обеспеченности витаминами и микронутриентами у детей с РАС изучен недостаточно, описаны только единичные наблюдения специалистов. Для объективной оценки обеспеченности микронутриентами, а также влияния приема витаминно-минеральных комплексов на организм детей с РАС требуется проведение крупномасштабных исследований.

Всем детям с аутизмом рекомендовано придерживаться принципов рационального питания, с ежедневным потреблением достаточного количества овощей, фруктов, мяса, рыбы, круп и исключением из рациона легкоусвояемых углеводов, продуктов с высоким содержанием соли. Кроме того, им необходима адекватная коррекция недостаточности витаминов и минеральных веществ с помощью регулярного использования обогащенных либо специализированных пищевых продуктов витаминно-минеральных комплексов [11, 12, 29, 33].

По результатам ряда последних исследований, комплексная диетологическая коррекция приводит к улучшению показателей состояния питания, невербального IQ, симптомов аутизма и других проблем у подавляющего большинства детей с аутизмом [15, 34, 56].

При отсутствии коррекции и лечения нарушений пищевого поведения симптомы никогда самостоятельно не проходят, а только усугубляются. Терапия расстройств питания должна проводиться после оценки общего состояния здоровья, одновременно с лечением патологии ЖКТ и коррекцией нутритивного статуса [1, 27, 28, 33, 57-59].

К сожалению, в литературе нет сведений о наблюдениях за детьми с РАС в организованных детских коллективах образовательных организаций. Несмотря на это, дети с особыми возможностями здоровья, к которым относятся дети с РАС, сейчас обучаются в общеобразовательных учреждениях. В связи с этим вопрос об организации питания детей с РАС в образовательных организациях остается открытым.

Заключение

РАС - сложное мультисистемное заболевание, на развитие которого влияют многие взаимосвязанные между собой нейробиологические процессы, протекающие в организме. Усугублению симптомов заболевания способствуют нарушения в соматическом статусе ребенка, что подтверждает необходимость комплексного подхода в терапии РАС.

Среди детей с РАС имеются значительные отклонения в сторону как увеличения, так и уменьшения массы тела. Нарушения физического развития у детей с РАС могут быть связаны как с патологией ЖКТ, так и с нарушениями пищевого поведения.

Показано, что у детей с РАС нарушения пищеварительной системы сопровождаются воспалительными явлениями в ЖКТ, изменением иммунного статуса и вегетативного тонуса нервной системы и дисбактериозом.

Особое место в клинике РАС занимает пищевое поведение, которое, с одной стороны, приводит к нарушению нутритивного статуса ребенка, с другой - усугубляет течение патологии ЖКТ Коррекция нутритивного статуса и организация рационального питания ребенка с РАС является важным аспектом лечения детей с РАС. При этом весьма актуальным остается вопрос разработки рекомендаций по организации питания детей с РАС в образовательных учреждениях.
