The study of the informativeness and reliability of the healthy eating index for assessing of dietary peculiarity and eating behavior of Russian population
AbstractFor the integral assessment of diet, tools have been proposed, called nutritional quality indices, and, as separate options, healthy eating indices (HEI), which assess diets in relation to compliance with existing national recommendations for healthy eating for the population.
The aim of this work was to investigate the suitability and reliability of the developed baseline HEI for assessing the nature of the diet, modifications of the diet and eating behavior of the population.
Methods. To analyze the suitability and reliability of the HEI for assessing the features of dietary intake and eating behavior of the population, the data on the actual nutrition and eating behavior of adults 19+ years of both sexes obtained by Rosstat in 2013 and 2018 have been used. Along with using the 24-hour food replay method to study actual food intake, the questionnaires included a form for assessing the frequency of consumption of major food groups in a standard format: daily or several times a week, once a week, several times a month, once a month or less often, practically do not consume.
Results. The average HEI values for women in all food consumption groups are significantly higher than for men (p<0.01). Changes in average HEI values depending on the level of consumption of various products are multidirectional. When foods such as grains, vegetables and fruits are consumed in quantities greater than the median, the HEI is significantly higher than when consumed at less than the median level. On the contrary, when consuming more than the median of meat products, fatty products and confectionery products, as well as for those who consumed sausages, the HEI for both women and men is significantly lower than for those who consumed less than the median or did not consume these products. The average HEI values in men and women who consumed cottage cheese, kefir or yogurt are significantly higher than among those who did not consume these foods. Significantly higher HEI values were found in vegetarians compared to non-vegetarians. The average HEI values of adults of both sexes in autumn are significantly higher than in spring, which is confirmed by the significantly higher consumption of vegetables and fruits in autumn, which are com-ponents-indicators of the integral HEI. The dependence of the HEI values on the frequency of consumption of the main food groups, which are its components or affect the consumption of components, has been shown. The higher the frequency of consumption of meat products, butter, salad dressings, chocolate and sweets or sweet carbonated drinks the lower is the HEI. On the other hand, the higher the frequency of consumption of vegetables, fruits, milk, fermented milk, cottage cheese or curd the higher is the HEI.
Conclusion. The changes in the values of the HEI at various values, as well as the frequency, of food consumption indicate the possibility of HEI using as a marker for assessing the relationship between diet, health status and the incidence of chronic noncommunicable diseases.

Результаты анализа ИЗП в зависимости от частоты потребления различных групп и подгрупп продуктов представлены в табл. 6. Частота потребления хлеба, круп, макарон, соков и нектаров не влияет на величину ИЗП. Однако величина ИЗП прямо зависит от частоты потребления овощей, фруктов, молока и кисломолочных продуктов, сыров, творожных изделий, рыбы и морепродуктов. С другой стороны, величина ИЗП снижается при высокой частоте потребления мяса животных и птицы, переработанного мяса (мясных изделий вареных и копченых), салатных заправок (майонез, растительное масло), кондитерских изделий и сладких газированных напитков. Таким образом, частота потребления различных продуктов разнонаправленно влияет на величину ИЗП взрослого населения в зависимости от вектора изменений фактического потребления компонентов-индикаторов, составляющих ИЗП.

Обсуждение
ИЗП, включая понятие "качества", отражают не только положительную и поощряемую характеристику питания, но и должны включать совокупность и интегральную характеристику неблагоприятных алиментарных факторов риска ХНИЗ. Наряду с общим качеством рациона питания ИЗП находят применение для мониторинга характера питания и пищевого статуса населения, оценки эффекта (мониторинга) вмешательства в области питания и в эпидемиологии при исследовании связи между характером питания, пищевым поведением и риском различных ХНИЗ [10-14]. По данным [15, 16], ИЗП коррелировал с изменениями потребления макронутриентов населением США, отражая динамику снижения потребления сахара и увеличения потребления цельнозерновых продуктов за 1999-2016 гг. Таким образом, данные литературы свидетельствуют о возможности использования ИЗП для мониторинга и оценки характера питания и пищевого поведения различных групп населения.
Представленные в настоящей работе данные об изменении величин ИЗП в зависимости от модификации пищевого рациона или пищевого поведения следует в первую очередь оценивать с точки зрения конструкции разработанного нами ИЗП [9]. Следует иметь в виду, что ИЗП сконструирован из 2 групп компонентов-индикаторов. Первая группа - компоненты адекватности потребления основных групп пищевых продуктов рациона питания: мясные, молочные, зерновые, овощи, фрукты. Чем выше величины потребления этих продуктов, тем выше ИЗП. Другая группа компонентов-индикаторов ИЗП - критически значимые факторы риска ХНИЗ, потребление которых рекомендуется ограничивать, - компоненты ограничения потребления. Чем выше потребление общего жира, насыщенных жирных кислот, холестерина, добавленного сахара и добавленной соли, тем ниже ИЗП. Становится объяснимым повышение ИЗП при более высоком потреблении овощей, фруктов, зерновых и молочных продуктов у вегетарианцев, а также в осенний период года. Эти факты подтверждаются данными литературы, в которых показана положительная связь величины ИЗП с потреблением фруктов, овощей и бобовых [17]. Показано также, что рационы питания с высоким индексом качества питания характеризуются более высоким потреблением растительных продуктов, включая цельнозерновые продукты, низким потреблением красного и переработанного мяса, добавленной соли, сладких напитков, алкоголя и трансжиров [18].
Особо следует оценить влияние потребления мясных продуктов и, в частности, колбасных изделий на ИЗП. Мясные продукты, в том числе колбасные изделия, в рационе взрослых влекут за собой увеличение потребления общего жира, насыщенных жирных кислот, поваренной соли и холестерина, что отражается на величине (снижении) баллов, насчитываемых за счет этих компонентов (см. табл. 3). Аналогичным образом объясняется снижение ИЗП рациона при потреблении колбасных изделий по сравнению с отсутствием их в рационе. В связи с этим колбасные изделия были исключены из состава мясных продуктов при конструировании ИЗП, так как этот факт компрометирует адекватное потребление мясных продуктов как поощряемый компонентиндикатор ИЗП.
Значительное снижение ИЗП выявлено в группе респондентов, потреблявших больше медианного уровня жировые и кондитерские сахаристые изделия, что объясняется более высокими величинами потребления (в % общей энергии рациона) жира или добавленного сахара и меньшими баллами этих компонентов в ИЗП в соответствующих группах взрослых.
В этом же аспекте следует оценивать более высокие величины ИЗП у лиц обоего пола, выявляемые в осенний период обследования, по сравнению с весенним периодом (см. рисунок). Данные, представленные в табл. 5, характеризуют особенности среднесуточного рациона питания взрослых по сезонам года. Установлена статистически значимая разница в потреблении овощей и фруктов, и, как следствие, увеличение потребления β-каротина, пищевых волокон и витамина С ранней осенью по сравнению с весной. Основную роль в увеличении ИЗП осенью играет более высокое потребление овощей и фруктов как компонентов-индикаторов индекса.
Исследование частоты потребления групп продуктов у тех же респондентов, для которых проанализированы величины ИЗП, позволило показать зависимость ИЗП от характера пищевого поведения взрослого населения (см. табл. 6). Так, чем больше частота потребления мясных изделий, масла сливочного, салатных заправок, шоколада и сладостей, сладких газированных напитков, тем ниже ИЗП. С другой стороны, при более частом потреблении овощей, фруктов, молока и кисломолочных продуктов, творога и творожных изделий средние величины ИЗП значительно выше, чем при низкой частоте или отсутствии потребления данных продуктов.
Полученная статистически обоснованная зависимость ИЗП от частоты потребления групп продуктов предполагает возможность разработки вариантов ИЗП, основанных на анализе частоты потребления продуктов, как это реализовано в США и других странах [3, 19, 20]. Анализ частоты потребления пищи как метод оценки фактического питания и пищевого поведения существенно облегчает проведение исследования, а также позволяет оценить обычное привычное питание за определенный промежуток времени. Разработка конструкции и оценка достоверности ИЗП, основанного на анализе частоты потребления пищи, существенно облегчит и упростит его широкое использование.
Изменение ИЗП при различной величине, а также частоте потребления пищевых продуктов свидетельствует о возможности использования ИЗП как маркера оценки и мониторинга характера питания и пищевого поведения различных групп населения с целью исследования взаимосвязи питания, состояния здоровья и заболеваемости ХНИЗ.
