Genetically determined trehalase deficiency in various population groups of Russia and neighboring countries

Abstract

Due to the low specificity and sensitivity of non-invasive clinical tests trehalose malabsorption remained out of sight of gastroenterologists. Therefore, the specialists regard this disorder as rare. Trehalose became widely used in the food industry as a harmless sucrose substitute, sweetener and stabilizer. After the discovery of the trehalase gene (rs2276064 TREH), it was found that the A*TREH allele is the determinant of the disaccharide absorption disorders, and the allele’s carriership may be high in some groups. There is not enough information on the A*TREH frequency in the population of Russia.

The aim of the study was to analyze the allele and genotype frequencies of the trehalase gene (rs2276064 TREH) in the main population groups of the Russian Federation and neighboring countries.

Methods. DNA samples from 1146 unrelated subjects belonging to 21 population groups of Russia, Azerbaijan, Tajikistan and Mongolia were genotyped by the two following methods: 1) using the Infinium iSelect HD Custom Genotyping BeadChip (Illumina, USA) on the iScan platform; 2) by the real time polymer-chain reaction (PCR) method on the Bio-Rad CFX96 Touch amplifier.

Results. It has been found that on the territory of the Russian Federation the frequency of the A*TREH allele increases from the west to the east. The frequencies are lowest in the groups of Russians and Finns of the Northwest (0.01-0.03), up to 0.07 in the populations of Central Russia and the Volga region, and even higher toward the Southern Urals (Bashkirs 0.15), in the Transurals and Southern Siberia (0.19 in the Altai people, 0.30 in the Tuvinians and Mongols). Up to 1% of the population of the European part of the Russian Federation have the AA*TREH genotype (i.e. trehalose intolerance in phenotype), and up to 15% (GA*TREH genotype) have a reduced ability to absorb the disaccharide. In the Asian part of the country (Siberia, Altai, Baikal) the genotypes carriers constitute up to 12 and 46% respectively.

Conclusion. Trehalose malabsorbtion is an underappreciated problem of particular practical importance for regions with high concentrations of indigenous population (Yakutia-Sakha, Buryatia, Tyva, etc.). It would be feasible to consider food labelling of trehalose.

В результате были сформированы выборки, позволяющие судить об общих закономерностях изменчивости частот аллелей и генотипов TREH в популяциях основной части территории России и некоторых сопредельных государств (см. табл. 2).

Обсуждение

Опубликованные в доступных нам работах данные о частоте встречаемости аллеля A*TREH приведены по материалам отдельных выборок, объем которых в большинстве случаев не достигает и 10 человек [16-18]. Наши предыдущие исследования в популяциях коми и шорцев дали информацию о географически локальных группах и лишь отчасти дополнили складывающуюся картину [19, 20]. Таким образом, распространенность трегалазной энзимопатии и генетических детерминант активности трегалазы у населения РФ до сих пор практически не изучена. Учитывая скудность материалов для сравнения, мы вынуждены ограничиться рассмотрением только результатов настоящего исследования, приведенных в табл. 2.

Анализ показывает, что в целом на территории РФ частота носительства аллеля A*TREH нарастает с запада к востоку.

Самые низкие частоты аллеля A и генотипа AA*TREH установлены в группах русских и финнов СевероЗапада, с незначительным повышением в популяциях Центральной России и Поволжья. Так, русские Архангельска отличаются от остальных русских и восточнославянских выборок по частотам аллелей (р=0,04; значимых различий по генотипам нет). В восточном направлении рассматриваемые частоты постепенно нарастают к Южному Уралу и Зауралью: отличия русских от башкир и казахов по частотам аллелей и генотипов р<0,0001. Нарастание частот аллеля A сохраняется в направлении Южной Сибири. Отличия башкир от алтайцев незначимы, но от группы народов Южной Сибири (тувинцев-тоджинцев, тофаларов, шорцев) башкирская выборка отличается достоверно (р<0,0001).

В терминологии физической антропологии эту картину можно описать как увеличение частот аллеля A и генотипа AA*TREH по мере нарастания монголоидного расового компонента. При этом даже внутри самой монголоидной расы наблюдается градиент увеличения частоты к востоку, например, при переходе от групп населения Южной и Центральной Сибири к популяциям Дальнего Востока.

С этим полностью согласуются данные, характеризующие выборки народов Кавказа, Закавказья (Азербайджан) и Средней Азии (Таджикистан). В этих европеоидных группах носительство аллеля G практически совпадает с обнаруженным в таких же европеоидных популяциях русских, украинцев и белорусов.

В близкую по частотам аллелей и генотипов группу входят представители южносибирской расовой группы, переходной от большой европеоидной к монголоидной: башкиры, алтайцы, казахи, шорцы, тувинцы. Они значимо отличаются от объединенной группы русских, украинцев и белорусов по частотам аллелей и генотипов (р<0,001 во всех случаях).

По сравнению с выборками Южной Сибири и казахов (AA*TREH 0,08 и 0,07 соответственно) частота генотипа AA* в монгольской выборке (0,12) сохраняет тенденцию к возрастанию, хотя различия и не достигают уровня статистической значимости.

Заключение

Полученные нами данные согласуются с накапливающейся информацией о распределении генетических детерминант метаболизма трегалозы в различных популяциях мира [16-18], а также с материалом, который постепенно поставляют клинические исследования [1, 8, 10].

Молекулярно-генетические исследования показывают, что первичная (генетически детерминированная) трегалазная недостаточность распространена значительно шире, чем это можно было предположить на основании клинических наблюдений. В отличие от других энзимопатий (в частности, генетически детерминированной недостаточности лактазы), нарушения усвоения трегалозы проявляются в разных формах: умеренно сниженной абсорбции в кишечнике (мальабсорбции) у носителей генотипа GA*TREH и выраженной непереносимости у гомозигот AA*TREH [4].

Полученные нами материалы свидетельствуют, что в европейской части РФ около 1% населения имеет генотип AA*TREH, детерминирующий непереносимость трегалозы, а до 15% (носители генотипа GA*TREH) могут характеризоваться сниженной способностью к усвоению этого дисахарида. В группах азиатской части страны (Сибирь, Алтай, Забайкалье) соответствующие риски достигают 12 и 46%. Результаты представляют особое практическое значение для регионов с преобладанием или высокой концентрацией коренного населения, например республик Саха (Якутия), Бурятия, Тыва и т.д. Эта информация должна быть доведена до специалистов в области пищевых производств, гастроэнтерологов, диетологов, нутрициологов. Мы полагаем, что высокая распространенность частичной и полной непереносимости трегалозы, клинически проявляющейся в диспептических симптомах разной степени выраженности, требует рассмотрения вопроса о нанесении на пищевые продукты и напитки маркировки, извещающей о наличии соответствующих добавок.

Представленные в настоящей публикации данные могут служить базой для более детального изучения распространенности генетически детерминированной мальабсорбции и неспособности к усвоению трегалозы в различных группах населения России.
