Eating behavior and skills of premature children in different age periods

Abstract

Premature infant feeding often causes a number of difficulties not only during the nursing of a baby in the neonatal period, but also in further periods of the formation of nutritional skills and habits. Eating difficulties are noticeable not only during the first year of life. The further development of the eating behavior of premature children differs from full-term peers.

The aim of the publication is to demonstrate the peculiarities of eating skills and eating behavior in premature infants throughout the entire period of childhood.

Material and methods. The search for original studies and systematic reviews that evaluated eating skills or eating behavior in children and adolescents born prematurely at various age periods was carried out using the eLIBRARY.ru and PubMed databases.

Results. Children with a gestation period of less than 32 weeks at birth are most vulnerable to the development of eating disorders, since the suck-swallow-breath pattern is in the process of maturation. Eating difficulties are noticeable during the first year of life, since the formation of eating behavior is closely related to the development of motor skills and the neurodevelopment. The proportion of premature children experiencing nutritional difficulties decreases with age, but does not become comparable with that among full-term peers. Later with time, motor disorders are replaced by selective appetite, and tendencies to eating disorders are formed. In addition, with age, gender differences in eating behavior appear, which are characterized by an increase in eating disorders among boys.

Conclusion. Currently, the data demonstrating characteristic age patterns in children born prematurely have been accumulated. Such a long-term effect of premature birth on eating behavior dictates the need for close attention to this group of children among pediatricians and the development of specialized programs for monitoring of preterm children.

Особенности характера питания, его нарушений в школьном возрасте у недоношенных детей практически не исследованы. По собственным данным, недоношенные дети в подростковом возрасте имеют отличительные черты пищевого поведения согласно опроснику Dutch Eating Behavior Questionnaire (DEBQ) [39]. Согласно опроснику выделяют 3 вида нарушений пищевого поведения: ограничительный, эмоциогенный и экстернальный. Эмоциогенный тип характеризуется избыточным потреблением пищи в ответ на эмоциональные переживания, экстернальный тип - избыточным потреблением пищи при воздействии внешних раздражителей (запах, вид еды). Ограничительный тип проявляется избыточным самоконтролем и ограничением в потреблении пищи, однако после ограничения происходят срывы в виде переедания. Так, среди мальчиков, рожденных преждевременно, чаще отмечались нарушения пищевого поведения по сравнению с доношенными сверстниками. У мальчиков, родившихся недоношенными, баллы по шкалам ограничительного и эмоциогенного типов пищевого поведения выше, чем у их доношенных сверстники (р<0,01). При этом среди недоношенных детей чаще нарушения пищевого поведения фиксировались у мальчиков (р<0,01). Девочки, напротив, реже имели нарушения пищевого поведения по сравнению с доношенными сверстницами (р<0,01) [39].

Постепенно накапливаются данные об отдаленном влиянии недоношенности на пищевое поведение взрослого населения. Группа финских ученых активно изучала влияние факт преждевременного рождения на пищевое поведение и восприятие собственного тела у взрослых (средний возраст - 24 года), согласно опроснику Eating Disorder Inventory 2 (EDI-2). Так, по результатам опросника мужчины не имели различий в зависимости от срока гестации при рождении. Женщины со сроком гестации при рождении <34 нед имели значительно меньше симптомов, связанных с нарушением питания, по сравнению с рожденными в срок сверстницами. Авторы предположили, что у данной категории взрослых существует некий протективный фактор [40].

Существуют данные о предпочтении продуктов у взрослых, родившихся недоношенными. Американскими учеными была выдвинута гипотеза о пищевых предпочтениях взрослых, родившихся недоношенными, на основе результатов предыдущих исследований по изучению диетических предпочтений среди детей и взрослых, родившихся со ЗВУР независимо от срока гестации при рождении. Так, сообщалось, что дети и взрослые, родившиеся с ЗВУР, предпочитают сладости, соленый вкус, больше потребляют углеводов и при этом меньше потребляют продукты, богатые белком, фрукты и овощи. Была выдвинута гипотеза о вышеуказанных предпочтениях в питании у тех взрослых, которые были рождены недоношенными, как о факторе риска сердечно-сосудистых заболеваний [41]. Лонгитюдное исследование предполагало наблюдение недоношенных детей с рождения, и к возрасту 24 лет (n=129) была проведена оценка образа жизни, характера питания, биохимических и других параметров. В результате у взрослых, родившихся недоношенными, были установлены пищевые предпочтения, подобные тем, что выявлялись у лиц, рожденных с ЗВУР, - склонность к сладкой пище, меньшее потребление белковых продуктов, овощей и фруктов, а также склонность к однообразной пище [41]. Подобные пищевые предпочтения, в свою очередь, были связаны с повышенными биохимическими и антропометрическими факторами риска сердечно-сосудистых заболеваний.

Заключение

Таким образом, в настоящее время накоплены данные, демонстрирующие долгосрочное и, вероятно, пожизненное влияние преждевременного рождения на пищевое поведение человека. Доля детей, родившихся недоношенными, испытывающих трудности в питании, с возрастом уменьшается, однако все же не становится сопоставимой с таковой среди доношенных сверстников. Отмечаются некие возрастные паттерны, характерные для недоношенных детей. Так, в периоде новорожденности нарушения питания характеризуются в большей степени оромоторной дисфункцией, низкой толерантностью к пище, отказом от питания, что обусловлено морфофункциональной незрелостью органов и систем. Далее, в первые 2 года жизни, оромоторные нарушения могут сохраняться и может возникать ограничительный/избирательный характер питания, который, вероятно, обусловлен практикой кормления. С увеличением возраста возникают отличительные гендерные черты в пищевом поведении. При этом мальчики, рожденные недоношенными, образуют группу риска по формированию трудностей в кормлении и развитию пищевых расстройств. Гендерные особенности пищевого поведения у недоношенных детей диктуют необходимость учета пола в оценке питания и индивидуального подхода в коррекции.

В настоящее время отсутствуют специализированные программы по профилактике пищевых нарушений и снижению рисков развития метаболических расстройств у недоношенных детей. Знание и понимание особенностей пищевых навыков и поведения недоношенного ребенка могут служить основанием для улучшения и персонализации подходов к выхаживанию недоношенного ребенка как на первом году жизни, так и в дальнейшие возрастные периоды.
