Specialized vitamin-mineral supplements for persons in extreme conditions

Abstract

Monitoring the actual nutrition of various groups of the population of the Russian Federation indicates the presence of pronounced deficiencies of essential micronutrients, and above all vitamins and biologically active compounds. Deficiency of many micronutrients is a risk factor for the development of a number of conditions (anxiety, depression, etc.) and non-communicable diseases (cardiovascular, cognitive and neuromuscular disorders). It reduces the effectiveness of the treatment of injuries and wounds, and also leads to a negative effect on the antioxidant protection of the body and a more pronounced response to stress. To prevent long-term consequences, the population is primarily provided with psychological assistance, not always paying due attention to healthy nutrition.

The aim of the research was to substantiate and develop vitamin-mineral supplements (VMS) for special purposes to restore impaired functions in various population groups in extreme situations, including those located in the zones of a special military operation.

Material and methods. A review of the literature on the problem in recent years was carried out using the databases of the RSCI, Google Scholar, ResearchGate, PubMed by the keywords “B vitamins”, “vitamin-mineral supplement”, “military personnel”, “mood”, “efficiency”, “depression”, “refugees”, «multivitamin», «conflict», “efficiency”.

Results. In an emergency situation, under conditions of high physiological (physical and neuropsychiatric) stress, the requirements in micronutrients increase. An inverse association between micronutrient status and the development of symptoms of depression, sleep disturbance has been found. The analysis of the data on the administration of VMS with different composition in the nutrition of the population and law enforcement officers showed the effectiveness of its intake for all persons in the zone of military conflicts. At the same time, VMS should contain a complete set of vitamins, in doses for B vitamins 200–300% of the recommended daily intake (RDI), vitamin D and other vitamins in a dose of 100%, magnesium, zinc, iodine, iron – in doses up to 50% of RDI. The administration of such VMS for 1–6 months provides an increase in blood serum vitamin level and antioxidant activity, leads to an improvement in functional adaptation and military professional performance, increases self-esteem of health, reduces symptoms of stress and anxiety, improves the mood.

Conclusion. The expediency of including VMS for special purposes in the nutrition of both military personnel and the entire population in the zone of military conflict has been proved. Enrichment of the diet with micronutrients is a reliable non-drug prevention of health disorders caused by stress in emergency.

Микронутриенты присутствуют в пище в небольших количествах, действуют согласованно, взаимодействуя друг с другом и другими пищевыми веществами. Взаимодействие витаминов заключается прежде всего в том, что каждый витамин в организме должен превратиться в свою активную (чаще всего коферментную) форму под действием ферментов, активность которых зависит от обеспеченности другими витаминами [47].

Для проявления влияния на биохимические и физиологические процессы в организме требуется время ожидания (в зависимости от микронутриента от короткого до длительного) [48]. Прием более высоких доз витаминов позволяет достичь оптимизации микронутриентного статуса организма за более короткий срок, а наличие в составе ВМК всех витаминов даст возможность быстрее восстановить до адекватного уровня обеспеченность организма витаминами из состояния множественной микронутриентной недостаточности [27]. Именно для этих целей применимы ВМК с высоким содержанием витаминов, составляющим до 300% от физиологической потребности (для витаминов С и Е до 1000%), но не превышающим верхний допустимый уровень потребления в составе специализированной пищевой продукции. Увеличенная потребность в витаминах группы В у военнослужащих вследствие повышенных энерготрат [49] также может быть удовлетворена за счет дополнительного приема ВМК с их высоким содержанием.

Анализ данных (см. табл. 1) свидетельствует о целесообразности включения в состав ВМК специального назначения для лиц, находящихся в условиях специальной военной операции по защите Донбасса, полного набора витаминов, причем витаминов группы В в дозах 200-300% от РНП, витамина D и остальных витаминов в дозе 100% от РНП, а также магния, цинка, йода, железа - в дозах до 50% от РНП (табл. 2).

Что касается витаминов-антиоксидантов, был сделан вывод о том, что использование витаминов А и Е в повышенных дозах, приближающихся к верхнему безопасному уровню потребления [50], населением, не имеющим дефицита этих микронутриентов, нецелесообразно [51]. При этом подчеркивается, что этот вывод не распространяется на высокое потребление фруктов и овощей - источника природных антиоксидантов, которое считается безопасным и полезным [52].

Заключение

При создании рецептуры специализированного пищевого продукта чрезвычайно важно не только обоснованно подобрать его композиционный состав, но и выбрать эффективные дозы биологически активных ингредиентов, обеспечивающие коррекцию недостаточного потребления микронутриентов.

Витамины являются взаимосвязанными, синергетическими микронутриентами, полный потенциал которых достигается, когда они находятся в правильном соотношении друг с другом. Для устранения недостатка микронутриентов необходим прием ВМК с полным набором витаминов и рядом минеральных веществ [53].

При множественной микронутриентной недостаточности дополнительный прием многокомпонентных ВМК с высокой вероятностью окажется полезным для поддержания адаптационного потенциала организма, неврологического и физического здоровья населения и участников боевых действий в ходе специальной военной операции по защите Донбасса. Заживление ран и травм также может быть ускорено за счет оптимизации микронутриентного состава рациона [54].

Обосновано включение в состав ВМК специального назначения для лиц, находящихся в условиях специальной военной операции, полного набора витаминов, причем витаминов группы В в дозах 200-300% от РНП, остальных витаминов - в дозе 100% от РНП, а также магния, цинка, йода, железа - в дозах до 50% от РНП.

В условиях стрессовых ситуаций для поддержания микронутриентного статуса и адаптационного потенциала организма целесообразно применять многокомпонентные ВМК как в качестве самостоятельного продукта, так и путем включения их в состав специализированной пищевой продукции специального назначения, что будет способствовать функциональной активности органов и клеток иммунной системы. Поддержание оптимальной обеспеченности организма микронутриентами обеспечит физическую и умственную работоспособность, стрессоустойчивость, особенно если причиной их снижения был недостаток того или иного микронутриента в рационе.
