Influence of environmental factors and polymorphic loci rs6580502 of the SPINK1 gene, rs10273639 of the PRSS1 gene, rs213950 of the CFTR gene on the risk of developing acute alcoholic-alimentary pancreatitis
AbstractThe study of genetic and environmental factors on the risk of acute alcoholic-alimentary pancreatitis (AАAР) is especially relevant to interpret individual links of pathogenesis, to reduce the incidence by eliminating the impact of harmful factors and improve the quality of life of the population through the introduction of optimal nutrition, and a healthy lifestyle, which is especially important for carriers of risk genotypes.
The aim of the research was to study the influence of environmental factors and polymorphic loci rs6580502 of the SPINK1 gene, rs10273639 of the PRSS1 gene, rs213950 of the CFTR gene on the risk of АAР.
Material and methods. Blood DNA samples obtained from 547 patients with AАAР and 573 healthy individuals were used as the material for the study. The groups were comparable by sex and age. All participants were assessed qualitatively and quantitatively for risk factors, smoking and alcohol consumption, the frequency, quantity and regularity of intake of various types of foods, as well as the size and number of portions eaten. Genomic DNA was isolated by the standard phenol-chloroform extraction method, multiplex genotyping of SNPs was performed on a MALDI-TOF MassARRAY-4 genetic analyzer.
Results. It was found that the T/T genotype (p=0.0012) of the rs6580502 SPINK1 was associated with an increased risk of AAAP, and the T allele (p=0.0001) and C/T and T/T genotypes (p=0.0001) of the rs10273639 PRSS1, A allele (p=0.01) and A/G and A/A genotypes (p=0.0006) of the rs213950 CFTR were associated with an decreased risk of the disease. The revealed effects of polymorphic loci of candidate genes were enhanced by the effect of alcohol consumption. The risk of AAAP was reduced by fat intake of less than 89 g per day in carriers of the A/G-A/A CFTR genotypes (rs213950), consumption of fresh vegetables and fruits of more than 27 g per day in carriers of the T/C-T/T PRSS1 genotypes (rs10273639), protein intake of more 84 g per day in carriers of T/C-T/T PRSS1 rs10273639 and A/G-A/A CFTR rs213950. The most significant models of gene-environment interactions included risk factors: deficiency in the diet of protein, fresh vegetables and fruits, smoking, and polymorphic variants of the PRSS1 (rs10273639) and SPINK (rs6580502) genes.
Conclusion. In order to prevent the development of AAAP, carriers of risk genotypes of candidate genes need not only to exclude or significantly reduce alcohol consumption (in terms of volume, frequency and duration); but also carriers of the A/G-A/A CFTR genotypes (rs213950) need to balance the diet by reducing fat intake to less than 89 g per day and increasing protein intake to more than 84 g per day; carriers of the T/C-T/T PRSS1 (rs10273639) genotypes should increase their intake of fresh vegetables and fruits over 27 g/day and protein over 84 g/day.

Количественная характеристика изученных алиментарных факторов риска для пациентов с ОААП (критерием был уровень значимости различий между группами согласно тесту Манна-Уитни) представлена в табл. 1.

Полученные медианы показателей для пациентов были использованы в качестве количественных критериев фактора риска в дальнейшем анализе взаимодействий генотип-среда.
Полученные результаты анализа совместного влияния полиморфных локусов изучаемых генов и средовых факторов, касающихся потребления алкоголя, на риск развития ОААП представлены в табл. 2.

Обнаружена связь SNP SPINK1 (rs6580502) с повышенным риском развития ОААП при употреблении алкогольных напитков чаще 2 раз в неделю. Установлена ассоциация полиморфных локусов генов CFTR (rs213950) и PRSS1 (rs10273639) с пониженным риском развития ОААП у пациентов, употребляющих алкоголь реже 2 раз в неделю. Также протекторный эффект наблюдали у носителей генотипов PRSS1 (rs10273639) при длительности употребления алкогольных напитков <10 лет.
Как видно из данных табл. 2, полиморфные варианты SPINK1 (rs6580502), CFTR (rs213950) ассоциированы с повышенным риском развития ОААП при употреблении алкоголя в объеме >200 г чистого этанола в сутки, а PRSS1 (rs10273639) - с пониженным риском развития заболевания при употреблении алкоголя в меньшей дозе.
Соблюдение правильного режима питания, оптимальный качественный и количественный состав рациона питания как неотъемлемые компоненты здорового образа жизни могут оказывать профилактическое воздействие, а нарушения приводят к развитию болезни [11, 15, 18, 19]. Результаты совместного влияния полиморфных вариантов изучаемых генов и показателей рациона питания на риск развития ОААП представлены в табл. 3.

Как следует из данных табл. 3, потребление жиров <89 г/сут у носителей генотипов A/G-A/A CFTR (rs213950), потребление свежих овощей и фруктов >27 г/сут у носителей генотипов T/C-T/T PRSS1 (rs10273639) и потребление белка >84 г/сут у носителей T/C-T/T PRSS1 rs10273639, A/G-A/A CFTR rs213950 снижали риск развития ОААП.
Далее были применены непараметрические статистические и биоинформатические подходы для оценки вовлеченности генетических и средовых факторов в полигенные механизмы развития ОААП путем моделирования 2- и 3-уровневых межгенных и генно-средовых взаимодействий, детерминирующих предрасположенность к заболеванию.
На основании количества статистически значимых mbmdr-моделей был рассчитан соотносительный вклад каждого изученного генетического маркера и различных средовых факторов риска в предрасположенность к развитию ОААП. Общий вклад средовых факторов в генно-средовые взаимодействия составил 43,8%, а среди средовых факторов, имеющих наибольшее этиологическое значение для развития ОААП, было сниженное потребление белка (18,4%), свежих овощей и фруктов (14,8%).
В табл. 4 представлены результаты наилучших n-уровневых mbmdr-моделей межгенных и генно-средовых взаимодействий (представлены только ассоциации, установленные после поправки на множественность тестов процедурой FDR), статистически значимо ассоциированных с развитием ОААП. Наиболее значимые модели генно-средовых взаимодействий при ОААП включали следующие факторы риска: сниженное потребление белка, свежих овощей и фруктов, курение и полиморфные варианты генов PRSS1 rs10273639 и SPINK rs6580502.

Заключение
В ходе исследования была установлена связь генотипа Т/Т (p=0,0012) rs6580502 гена SPINK1 с повышенным риском развития ОААП; аллеля Т (р=0,0001) и генотипов С/Т и Т/Т (р=0,0001) rs10273639 гена PRSS1 и аллеля A (p=0,01) и генотипов A/G и A/A (р=0,0006) rs213950 гена CFTR с пониженным риском развития ОААП. Частота генотипа Т/Т SPINK1 C/T rs6580502 была выше у мужчин с ОААП (р=0,008). Протекторный эффект генотипа А/G CFTR rs213950 наблюдали у пациентов обоего пола, но у женщин он был более выраженным (р=0,008), чем у мужчин (р=0,08). Частоту генотипов C/T и T/T PRSS1 rs10273639 наблюдали выше среди здоровых индивидов мужского пола (р=0,004), у женщин только генотип Т/Т чаще встречался в группе сравнения (р=0,005).
Обнаруженные эффекты полиморфных локусов генов-кандидатов усиливались действием алкоголя.
Потребление жиров <89 г/сут у носителей генотипов A/G-A/A CFTR (rs213950), потребление свежих овощей и фруктов >27 г/сут у носителей генотипов T/C-T/T PRSS1 (rs10273639), потребление белка >84 г/сут у носителей T/C-T/T PRSS1 rs10273639, A/G-A/A CFTR rs213950 снижали риск развития ОААП.
Наиболее значимые модели генно-средовых взаимодействий при ОААП включали следующие факторы риска: сниженное потребление белка, свежих овощей и фруктов, курение и полиморфные варианты генов PRSS1 rs10273639 и SPINK rs6580502.
Таким образом, с целью профилактики развития ОААП носителям рисковых генотипов необходимо не только исключить или значительно снизить употребление алкоголя (по объему, частоте и длительности), но и носителям генотипов A/G-A/A CFTR (rs213950) сбалансировать рацион питания за счет снижения потребления жиров <89 г/сут и увеличения потребления белка >84 г/сут; носителям генотипов T/C-T/T PRSS1 (rs10273639) увеличить потребление свежих овощей и фруктов >27 г/сут и потребление белка >84 г/сут.
